To ensure smooth processing of your loan applications, please submit the complete set of required forms and documents corresponding to your borrower category:

• Data Privacy Notice Form
• Health Declaration Form
• Photocopy of ID
• Latest Payslip

Please submit the following six (6) requirements:

• Pension Loan Form
• Data Privacy Notice Form
• Health Declaration Form
• Fortune Life Insurance Form
• Photocopy of ID
• Latest Payslip
