- Housing Loan Form
- Latest BJMP payslip (Photocopy)
- BJMP ID (Photocopy)
- Transfer Certificate of Title/Condominum (TCT/CCT) in the name of borrower, duly annotated by the Registry of Deeds/MORTGAGE ANNOTATION, IN FAVOR OF BJMP-MPC
- Loan and Mortgage Agreement (4 copies)
